Passports
Original Divorce Decree of Absolution with Official Seal, if one or both parties have been divorced
Original Death Certificate in the case of a widow or widower
Original proof if name changed by Deed Poll
If aged under 18 years, evidence of consent of parents is required in the form of an Original Affidavit, stamped by a Notary Public
If any of the required documents are not in English, an authenticated translation must be available
It is recommended that copies of all documentation be sent at least 3 weeks in advance to expedite the license application process.
